ADD FACILITY DATA TO BOX 32 FOR MEDICARE

 

 

Medicare, in some states, have started requiring that the clinic information also be printed in Box 32 of the HCFA form.  This box was formerly reserved for outside lab and services   information, or if the billing address of the clinic was different from the physical location. Please note that with version 4.2 or higher, box 32 will automatically fill in with the business name and address if this address is different from the clinic information.

 

Manually Adding these Fields to HCFA Form

The easiest way to edit this form is to do this from billing.  In this way, we can test to be sure that everything is doing what we need.  Once in billing, pick a patient with which to test.  It is best if the patient already uses the form we are editing so be sure to check for either the HCFA Red Med 1.7 or the HCFA Red Med-WA 1.7 format.  Now click on EDIT.  This brings up a screen with all the small “boxes”.

At the bottom of the screen (use the “elevator” on the right to move to the bottom of the format) you will see the group of boxes in the lower right that represents box 33.

 

Click on the first box that represent the Clinic Name.  You will see small "dots" to show that this field has been selected.  Now hold down the SHIFT key on your keyboard and click on the remaining 2 boxes that make up the address and city state zip areas.  When all three boxes are selected, hold down the CTRL key and press the letter "C" to copy (NOTE: you will not see anything happen when you do this action.  Now, continue to hold down the CTRL key and press the letter "V" to "paste".  You will see another set of these boxes laying somewhat over the top of the original ones.  Using your mouse, or your arrow keys, move these boxes to the left so that they are placed in the area reserved for Box 32.

 

Finally, hold down the CTRL key and press the letter “W” to save this format.
